Have you tried specifying the item in the stylesheet?

TYPE=TABFOOTING,ITEM=7,STYLE=BOLD,JUSTIFY=RIGHT,$

Think I got the right item number.

What format output is the report, you say DOC, but what is the FORMAT xxxx

With format HTMTABLE, you can use HEADALIGN=BODY, and use COLSPAN.

Another option is to try WIDTH for the items, You can specify how wide each item will be, but I don't think it is appropriate for this report.
