I have 4 prompts that I am trying to conditionally disable. The 2 Drop down lists (PLANT & BUYER) work but the 2 Text boxes (SUPPLIER & Part_Number) do not. Are these somehow handled differently? I am using the following logic.

if (REPORT[0].checked){
document.getElementById('PLANT').disabled=false;
document.getElementById('SUPPLIER').disabled=false;

} else if (REPORT[1].checked || REPORT[2].checked) {
document.getElementById('PLANT').disabled=true;
document.getElementById('SUPPLIER').disabled=true;
document.getElementById('BUYER').disabled=true;
document.getElementById('BUYER').disabled=true;
}

you can try this option instead of getElementById

document.form2.SUPPLIER.disabled=false;

This should work when the text box unique identifier must be "SUPPLIER".

On which button you are calling this javascript function like onClick of Submit button or is there any other event?

As your syntax appears to be OK, the first thing I would check is that you have assigned your text input controls the correct ID attribute value and case.
