use the Publish feature in MRE. it will produce an html launch page from your fex, turning all your parms into <input text... which you can then edit as you please to whatever kind of input elements you wish.

You can use either a CGI call or a servlet call to do this, but I have to agree with Susannah in that the easiest way may be to create it in MRE, publish it and steal the coding from there.
