My apologies if this question is either redundant or vague. I tried searching the forums before posting, but the nature of this problem, and my inexperience with WF, made it difficult to find relevant results.
I have created a report in WF that runs satisfactorily and am trying to create the accompanying HTML page. I created a new HTMLFORM, and imported my existing report. All appears well until the report is run, and HTML is selected from the 'report format' dropdown list. Doing so produces the following output:
When another file format such as PDF is selected from the list the output is fine, which leads me to believe that something is wrong with the HTML code itself. I have uploaded my code to pastebin:
I would be happy to provide any further information that may be useful, and I appreciate any assistance.

As far as I can tell, you shouldn't have "-HTMLFORM crp_query_web.htm" in the fex part of the html file because the HTML file itself (crp_query_web.htm) calls the fex in the iframe. Coding "-HTMLFORM crp_query_web.htm" brings in the HTML file a second time, resulting in JavaScript errors and the form objects displayed a second time.

Thanks for the reply!
I noticed that the problem also exists in the report itself when run outside of the web page. I found that -HTMLFORM line in the code on pastebin (line #231), and there is also an occurence of it in the underlying report code. I deleted it from the fex file itself and now the report runs correctly. Thanks!
I then deleted line 231 from my HTML code on pastebin, but the problem still exists when I run the report from the web page. Should I just recreate the HTML page now that the underlying report is working correctly?

Umm...I can honestly say that I don't know. Sorry for being a noobie, but I created this report and HTML page with the WF gui, not by writing code.

I followed my own advice and recreated the HTML page. When I imported the report with the edited fex file, the HTML page now works correctly...thank you, Francis!
