Using your output example for "England", I understand that you want to have "CAR:England" appear over the TITLES.
This can only be done if you put "CAR:England" in the HEADING but then you will have to use PAGE-BREAK on COUNTRY. Is that what you want?

Notice the use of null TITLES. Instead they are inserted in the SUBHEAD.
