Inability to support more than 2 calendar controls on a single form is a "known issue" with Developer Studio 532 and 533. Supposed to be fixed with 534.

Using more than 2 calendar controls on a single htm form is only supported when the client and reporting server WF versions are at 5.3.4, and Dev Studio 5.3.4 is used (though I wouldn't guarantee DS 534 is a valid requirement). The functionality of using more than 2 calendar controls was not available until our mid-tier was upgraded. The symptom is an IE session that runs without end.
